I have static DHCP assignments set up for IPv4 and IPv6:
But I can't get the IPv6 static lease to take effect:
I'm not sure what I am missing. Any help? Thanks.

You haven't applied the DUID in the static assignment.
Is it possible to match based on just the MAC address instead of a DUID?
(by the way, setting the DUID in the static lease fixed the problem)
I just found this: https://blogs.infoblox.com/ipv6-coe/dhcpv6-and-the-trouble-with-mac-addresses-part-1-of-2/
So I think the answer is no.
