It used to get a /64 delegated sometimes.

It would help to mention that you changed the ipv6 option in wan from manual to auto, so that we wouldn't have to try to understand what went wrong from 2 screenshots.
It stopped working in manual so as a last measures I switched to auto. It works 1 out of 10 times in either manual or auto.
Delegating a /64 is a red flag of poor ISP. I would not be surprised if the problem comes from their end.
Switch it back to manual, or assign the wan_6 in the wan firewall zone.
What you could do is to capture the dhcp6 packets to verify that you are not getting any response and raise the issue with your ISP.
