This is my best guess, I'm certain that CLK and CMD are correct (was able to decode them with a logic analyzer), D0 is still up in the air as I wasn't yet able to dump the chip.
There's also a RST on the bottom, but to get to get you have to grind down a via and solder to it. I can send a picture of it later.
