you could also try to add a 10k resistor between router tx and vcc, like mr3420:

and never connect vcc of serial adapter to anything..

Seems like your serial adapter has troubles to listen to the router. Is it a 3.3V model? Maybe get a different one.

rootfs seem to corrupted... it seems you have a mix of openwrt and ddwrt there...
Did you try to restore to openwrt or original firmware with tftp method? Refer to HERE

What?
If  you haven't connected your computer's TX (aka transmit)  to the router RX (receive)  how is the router going to "hear" what you're typing at it ?

And did you read the instructions from the wiki ?

- One issue could be that you're using a 5V device for 3.3 V router (i don't think that's a problem... but i haven't tried it with this router and it COULD be a problem )

- Did you check that part about the RX pin and the jumper ? (to which nebia88 linked directly)
