I just installed openwrt on my wrt3200acm using a PureFusion build, and I have set up cake sqm using the guidelines from this page. I just upgraded to this router, as my previous router did not have the cake qdisc available, and I am getting ping spikes still. I am connected via ethernet, and have connection speed of 70/18. What could I be doing wrong?

As for what im using to check for ping spikes, https://ping.canbeuseful.com/en#ping
My ping is completely unstable when looking at this, it spikes up to 100ms randomly, but pretty frequently.
Im with BT, and I believe im using VDSL, as we don't have fiber, but are living in a relatively new development (2 years old)
Try the Waveform bufferbloat/speed test mentioned above.
Waveform results look good.

Although this tool is reliable, the ping time measurement is indicative and not as accurate as the ping command. This is due to the fact that the tool uses the HTTP protocol(S).
I reset all my settings to default, swapped over to vanilla openwrt, and put my settings in again, just to make sure it wasnt my config. I currently am the only user connected to the network, I added my phone, and started streaming netflix, and my ping became instantly unstable. I ran that ping test on my pc, whilst doing the fast.com test on my phone. I haven't measured ping on my phone, as I only really care about ping stability on my pc

If this is a VDSL connection, I would set the Per Packet Overhead to 44.
Minimum = 12ms, Maximum = 66ms, Average = 27ms,

Nothing wrong with those numbers.
One test that I do (with no browsers involved) is a continous ping to a speed test server, while downloading a large file using wget (Linux distros run around 6 GB+).
Did you stop and restart SQM?
Yes, i did stop and start. I also noticed when i half the download speed in sqm settings, the ping spikes almost go away completely, is there anything else i can do other than this to prevent spikes?
You would need to determine where the latency is coming from.
Could be inside your network, inside your ISP's network, or outside the ISP's network.
I would download the free version of PingPlotter, and run tests to different endpoints.
It will give you a picture of what's going on along the route.

OK, that is good, you are using pppoe-wan in sqm.
Two things I forgot to ask for
a) a speedtest with sqm disabled
b) information from your modem about the sync
