Since iOS11, to improve security, iPhones will terminate USB data connections after one hour by default. This is easily changed by going to:

Settings > Touch ID/Face ID & Passcode > USB Accessories > ON

I wasted many hours trying to fix the constant disconnecting, would like to save everyone else the trouble.
